Enrollment Fee: $100 for individual, $150 for couple or family.

Home Care Visits - Let the doctor come to you, visits are in patient's home.
Distance within 5 miles from office. -
Patients with Medicare Part B- Sharon Direct Care will charge a monthly fee for services not covered under Medicare Part B and will arrange to seek reimbursement for services covered.

Cancellation Policy: 30 day written notice.

Annual Payment- pay annually and receive one month free. Enrollment Fee is waived if paying annually.

Referral refund- receive $100 off your next months' bill for any referred patient who joins Sharon Direct Care.
