I-693 Immigration Medical Exam
An important part of the immigration process is to have a medical exam performed with a USCIS certified Civil Surgeon. Dr. Bluestone and his staff will help guide you through the process and ensure that all parts of the exam and form are properly completed.
What to expect at the Medical Exam?
During your visit, Dr. Bluestone will review your vaccination records, medical history and perform a medical exam. The age-based required lab work (Syphilis, Gonorrhea, Tuberculosis) will be ordered, which can be done at a Quest Diagnostics location of your choice. Dr. Bluestone will also inform you of any vaccines that you are missing and that you are required to get.
Typical vaccinations required for adults include: Tdap, MMR, Varicella, Hepatitis B, Pneumococcal, Polio, Influenza (seasonal) and COVID-19.
Lab results typically take approximately one week to come back. Once they are received and everything is in order, we will have you return to the office to pick up the signed Form I-693. You will receive the signed form in a sealed envelope as required by USCIS and a copy for your own records. Please do not break the seal as it will be rejected by USCIS.
What to bring to your appointment?
Please bring the following items to your initial appointment:

Valid and current official picture ID (drivers license, foreign passport, employment ID)

Vaccination/Immunization records. If records are in a foreign language, they will need to be translated prior to your appointment.
