I-693 Immigration Medical Exam 

What is it?

An important part of the immigration process is undergoing a medical exam with a USCIS certified Civil Surgeon. We will help guide you through the process and ensure that all parts of the exam and the I-693 Form are properly completed. 

What to expect at the Medical Exam?

During your visit, the Civil Surgeon will review your vaccination records, medical history and perform a medical exam. The age-based required lab work (Syphilis, Gonorrhea, Tuberculosis) will be ordered, which can then be done at a Quest Diagnostics location of your choice. The Civil Surgeon will inform you of any vaccines that you are missing and required to get. â€‹

Lab results typically take one week to come back. Once they are received and everything is in order, we will have you return to the office to pick up the signed Form I-693. You will receive the signed form in a sealed envelope as well as a copy for your own records. Please do not break the seal as it will be rejected by USCIS. 

What is the cost to complete the Medical Exam and I-693?

The cost for the immigration exam and completed I-693 Form is $325.


Required labs- these are based on age and their costs are as follows:
Tuberculosis (QuantiFERON Gold)         $110
Syphilis                                                        $15
Gonorrhea/Chlamydia                              $40

Based on vaccine history some titers might be needed as well, costs are as follows:

Hepatitis B          $25

Varicella              $25

MMR                    $50


 
Please note, these prices do not include any necessary vaccinations, which can be obtained at local pharmacies and are typically covered by health insurance plans. We are able to help you find the most cost effective locations to obtain those vaccines.

We accept cash or credit card, and payment is required at the time of your first appointment.

What to bring to your appointment? 

Please bring the following items to your initial appointment:

  • Valid and current official picture ID (drivers license, foreign passport, employment ID)

  • Vaccination/Immunization records. If records are in a foreign language, they will need to be translated prior to your appointment. 

  • First 2 pages of the I-693 form. You can locate and print the most updated version of the form on the USCIS website by clicking here:
